AUTHORIZATION FOR RELEASE OF PERSONAL INFORMATION

The undersigned certifies that the information furnished in this application is true and correct. I authorize Jewelry Warehouse, or its agent, to verify any and all information submitted and to make any additional checks or background checks which it deems necessary or advisable.

 

I understand and state:

  1. That the information on this application is true to the best of my knowledge and that any misstatement is grounds for dismissal and liability.
  2. That I authorize Jewelry Warehouse to contact my former employers or other references for reference checks.
  3. If separated from this company for any reason and I have possession of company property, Jewelry Warehouse will withhold from my last paycheck the value of the property. In case of keys not immediately returned, Jewelry Warehouse will deduct the replacement costs of the locks. In the case of indebtedness to the company, Jewelry Warehouse will withhold the value of the indebtedness.
  4. If I handle the cash register, I understand that I am responsible for all register shortages. The shortage may or may not be reimbursed.
  5. I understand that the misrepresentation of, or omission of facts called for on this application is cause for cancellation of the application and/or separation from the company’s service, if I have been employed.
  6. As an employee of Jewelry Warehouse, I fully understand that all training techniques, procedures, and any and all information pertaining to the operation of Jewelry Warehouse or its subsidiaries are the sole property of Jewelry Warehouse, and shall be considered wholly proprietary thereto. Further, I pledge not to discuss or divulge any such proprietary information or knowledge that may compromise or in any way harm Jewelry Warehouse, with any party or individual either during or following my Employment with Jewelry Warehouse
  7. I understand my continued employment with Jewelry Warehouse – if hired – is based on successfully completing a 120 day probationary period.
  8. If I do not submit a 2 week notice prior to leaving my employment with Jewelry Warehouse and fail to work my scheduled hours during the notice period, I understand that I will make minimum wage for all hours worked on my last paycheck.

Nothing in this employment application and statement can be construed as a contract. The policies contained therein are merely guidelines. Both Jewelry Warehouse and the employee have the right to terminate the employment relationship at any time, with or without cause.

In the consideration of my employment, I agree to conform to the policies and procedures of the company.
